I believe you meant to say that typically it is best to smoke a brisket fat side up unless the smoker is very on the bottom of the meat.

Fat Up Fat Down?

Typically it's best to smoke a brisket fat side UP (not down), unless the cooker your brisket will be in is extremely hot directly beneath the brisket.

For instance, smoking a brisket in a Weber Kettle grill, it just might be better to smoke fat side down. The fat will protect the underside of the brisket from extreme heat.

Otherwise, fat cap on top keeps the entire brisket basted in fat as it cooks.
